How to Refer or Request Care
For Patients
Getting started is simple. Occupational therapy for lymphedema treatment requires a referral from your physician.
Step 1: Request a Referral
Contact your primary care provider or specialist and request an Occupational Therapy referral for “Evaluate and Treat.”
Step 2: Provide Information to Physician
To ensure your referral is processed quickly and correctly, please have your provider include one of the following diagnosis codes (if applicable):
Lymphedema: I89.0, Lipedema: E88.2 or R60.9, Chronic Venous Insufficiency: I87.0
Step 3: Have Your Provider Send the Referral
Your physician can submit your referral using one of the following:

Once your referral is received, you will be contacted promptly to schedule your initial evaluation.
